Low-Energy Stereotactic Radiotherapy for Treatment of Exudative Age-Related Macular Degeneration in a Treat-and-Extend Regimen.

چکیده
BACKGROUND AND OBJECTIVE To evaluate the effectiveness and safety of low-energy stereotactic radiotherapy (SRT) combined with anti-vascular endothelial growth factor (VEGF) treatment following a treat-and-extend regimen (TER) in wet age-related macular degeneration (AMD). PATIENTS AND METHODS Before/after SRT, the authors compared retrospective consecutive case series of 50 patients requiring frequent anti-VEGF treatment (every 4 or 6 weeks) in wet AMD, treated with a single session of SRT and TER (same manner pre/post-SRT). Outcomes were visual acuity (VA), recurrence-free interval, and central retinal thickness (CRT). RESULTS After SRT, CRT was reduced from baseline (407.3 μm ± 153.2 μm) to 12 months (320.2 μm ± 112.1 μm; P < .001), with statistical significance from month 2 onward. VA was stable for 12 months (64.0 letters ± 15.1 letters vs. 63.6 letters ± 16.2 letters). The mean recurrence-free interval increased from 4.24 weeks ± 0.66 weeks to 7.52 weeks ± 3.05 weeks at 12 months (P < .001). No severe side effects were observed. CONCLUSION Low-energy SRT, combined with anti-VEGF TER, was associated with reduced injection frequency and preserved VA during 12 months of follow-up. Retina specialists treating age-related macular degeneration recommend different approaches for patients than they would choose for themselves.
PURPOSE To evaluate the presence of cognitive biases among retina physicians when recommending treatment options for exudative age-related macular degeneration. METHODS Two random samples of retina specialists were surveyed regarding their treatment and dosing regimen choices among three anti-vascular endothelial growth factor biologics (aflibercept, bevacizumab, and ranibizumab). One group w...
متن کاملThe Study of Serum Asymmetric Dimethylarginine Concentrations in the Different Paraoxonase Phenotypes of Exudative Age-related Macular Degeneration Disease
Background and Aims: Age-related macular degeneration (ARMD) is a degenerative retinal disorder that causes progressive loss of central vision in older adults. The study aimed to determine the effect of asymmetric dimethylarginine (ADMA) as oxidizing metabolite and paraoxonase (PON1) activity within its phenotypes as an antioxidant agent in the development of such multifactorial disease. Mater...
متن کاملStereotactic radiotherapy for wet age-related macular degeneration: current perspectives
Neovascular age-related macular degeneration is a leading cause of blindness in the developed world. Currently, the treatment of choice is intravitreal injections of anti-VEGF medications. These require frequent dosing, up to monthly, and impose a substantial burden on patients and the health economy. Ionizing radiation was proposed as a possible treatment for age-related macular degeneration d...
متن کاملA 12-month prospective trial of inject and extend regimen for ranibizumab treatment of age-related macular degeneration.
PURPOSE To evaluate the safety and efficacy of a strictly applied inject and extend protocol for ranibizumab treatment of age-related macular degeneration. METHODS This is a prospective, multicenter, nonrandomized trial. Patients underwent standard induction with 3 intravitreal doses of 0.5 mg ranibizumab, each 1 month apart. Following this induction, patients were evaluated and received an i...

BACKGROUND To evaluate the clinical effectiveness and analyze the outcomes of a treat-and-extend (T&E) treatment regimen with ranibizumab for wet age-related macular degeneration (ARMD) in real life clinical settings over the first 2 years (24 months) of treatment. METHODS Retrospective analysis of visual acuity, spectral domain optical coherence tomography (SD-OCT) parameters and treatment b...
